     St. Anthony Foundation Awards Grants to 19 Milwaukee Area Agencies
 
 

The Archdiocese of Milwaukee has announced that the following agencies have received grants from the St. Anthony Foundation. The grants are issued to agencies that continue to serve the medically indigent in Milwaukee, through supporting programs and addressing their needs. The total funds awarded this year were $211,000.
The following is a list of the agencies receiving grants:

Benedict Center The Women's Harm Reduction Program offers a wide range of holistic services contributing to a healthy self and family, including Alcohol and Other Drug Addiction (AODA) treatment, case management, women's health, anger and stress management, parenting and nurturing, as well as basic and high school equivalency education.

Catholic Charities The Community Nurse - P.A.C.T. Family Resource Center offers a part-time nurse who provides triage care to ill children whose parents are unable to access the healthcare system because of language and financial barriers. The nurse also teaches families basic home care skills or assists the family in obtaining appropriate medical care should it become necessary.

Catholic Charities The Community Nurse - The Kids & Family Network Program was established to secure access to basic health care for school-aged children who are enrolled in near south side Catholic schools that serve the poorest geographic area of Milwaukee. The program also provides training for parents and teachers so they may be effective in their roles and present themselves as positive role models. The community nurse works full time as a case manager, meeting each child and each parent to target high-risk families that need guidance in the care and responsibility of the child.

Centro Primo A program was formed to educate women about the importance of good breast healthcare. Breast Healthcare Needs of Low-Income focuses on the people in the community that do not realize that early detection of breast cancer is extremely important to their overall health. They encourage women to have annual mammograms and to perform monthly breast self-exams. They work hard to expand services each year in hopes of meeting the needs of more women so that they will have easier access to the services.

Cross Lutheran Church The Bread of Healing Clinic is a program that is faith based. It provides medical care to those who are under insured or have no medical insurance, 94% of whom are African American. Cross Evangelical Lutheran Church is a diverse congregation, whose 800 baptized members include low and middle-income people, persons in recovery, persons from the gay and lesbian community, ex-offenders, and refugee families.

Guest House The Marquette Clinic for Women and Children is a collaborative clinic that utilizes the services of the Marquette University Nursing Staff and students, Guest House social service staff and Guest House clerical support staff. The clinic offers holistic nursing and wellness services free of charge to the community as well as the uninsured or underinsured. A social service worker identifies those referral sources most helpful for the patient to resolve the issues.

House of Peace The Community Nursing Center at UWM is located within the House of Peace community center. Residents of the House of Peace community do not have access to health care services or the means to finance health care costs. Lacking sufficient insurance to cover health costs, many report having become frustrated by recurring incidents of discrimination and abuse. In an attempt to respond to the need for greater access to primary health care within this community, the UWM House of Peace Community Nursing Center opened.

Interfaith Program for the Elderly The objective of the Bilingual Outreach Nursing program on the near south side is to prevent health crises and premature institutionalization through in-home assessments, follow-up visits and health care assistance for frail Hispanic elders. The outreach nurse also will attend to the needs of all older adults with the near south side boundaries as they are referred through the near south side Interfaith Office.

Marquette College Nursing The Urban Partners for Health: A Collaborative Project Between Marquette University College of Nursing and the central city community program focuses on primary prevention through health education, screening, and clinical care management. On-site services are provided, including early identification of health concerns affecting education achievement; individual consultation and care management for school children with health problems; group education regarding pertinent health and safety concerns.

Medical College of Wisconsin The unique partnership provided by Family House and the Medical College of Wisconsin Clinic serves several hundred residents in the inner-city community that would otherwise lack adequate health care access. Family House also provides non-medical related classes on money management, identifies job opportunities, and provides clothes and toys as part of its comprehensive care mission.

Rosalie Manor As a program of Rosalie Manor Community & Family Services, Supporting Today's Parents strengthens and empowers new parents, in a culturally competent way, toward healthy family growth, subsequently, decreasing child abuse and neglect in Milwaukee County. As a community based organization, Rosalie Manor offers Safe Places for youth between first through fifth grades, which allows students to remain at their school during after hours, and receive tutoring and recreational activities. Their programs serve all of Milwaukee County, but primarily serve economically disadvantaged families.

SET Ministry The SET Ministry provides for the community through three programs. With Family Empowerment, many families that are experiencing the challenges of low wages, limited access to health care, lifestyle related health problems and high infant illness receive SET’s services. The primary areas of focus and attention are prenatal care, immunizations, lead poisoning, lifestyle related health problems and parenting skills. Through AIDS Ministry, SET provides spiritual/emotional support for persons with HIV/AIDS, their families, friends and caregivers. Many of the clients are poor and have inadequate health and human resources. Through the program they are helped to make the proper connections. With Health Promotion Clinics, SET is able to serve the elderly and other adults with disabilities. Nurse teams provide health assessments, referrals for needed services and individualized health teaching.

STD Specialties Clinic, Inc. The STD Specialties Clinic provides health care to the marginalized populations of metropolitan Milwaukee. By virtue of the clinic's location, it is able to provide health care services to an ethnically and racially diverse population. The outreach programs serve the homeless and impoverished, and seek out individuals that are without the means or wherewithal to access traditional health care services. Due to a multitude of other factors, it is only through the clinic seeking out these individuals that they are tested and treated for a host of infectious diseases including HIV/AIDS.

St. Francis Foundation The Angel of Hope Clinic, located at the Hope House homeless shelter on Milwaukee's near south side, provides free health care services to homeless, poor, uninsured, and underinsured children and adults. The clinic serves individuals and families who are staying at the shelter or who live in the surrounding low-income neighborhood. The clinic's services help to empower people to improve their quality of life and to maintain a level of wellbeing that helps them to avoid catastrophic illness.

St. Francis Foundation The Parish Nurse Program serves hundreds of poor and low-income people each year through nine parishes on Milwaukee's near south side. The nurses assist individuals and families through home and hospital visits, telephone contact, health fairs, screening, and educational programs. The two parish nurses--one full time and one part-time--have been responsible for seniors receiving care, young families receiving support and assistance, and new networks of parish volunteers reaching out to people in need in their community. The nurses are bringing a heightened awareness of health and wellness to the community that will have long-term, as well as immediate, effects.

St. Joseph Hospital Community Foundation The Asthma Awareness Clinic recognizes the prevalence of unmanaged asthma among uninsured and underinsured individuals. In order to reach this community, the clinic provides its services to all patients regardless of the ability to pay. Working with the pharmacy department, the clinic also provides medication at no cost to individuals who qualify. The clinic's collaborative efforts include educational programs hosted at churches and daycare centers in the neighborhoods.

St. Mary's Foundation Milwaukee The Madre Angela Dental Clinic-Clinic Coordinator Project hires a clinic coordinator who is responsible for office and clinical staff, dental hygiene services, quality assessment, volunteer dentists and other professionals, as well as equipment and supplies. A major responsibility of the position is the challenge of evaluating the urgency of patient needs and scheduling care accordingly--a job that demands judgment based on both compassion and clinical expertise.

St. Mary's Foundation Milwaukee The Adult Learning Center Health Center Support Project makes use of the parish nurse from St. Mary's Hospital of Milwaukee to help the medically indigent who come to the Adult Learning Center. The center serves people with poor reading skills and participants lack the reliable income of long-term employment.

St. Mary's Foundation Milwaukee The St. Ben's Outreach Clinic provides homeless people in Milwaukee with primary and preventive health care services. The clinic facility provides the homeless with laboratory testing, clinical assessment, social work services, and on-site prescription dispensation. The Outreach Project brings health screening and health education to other locations that are easily accessible to the homeless and works to connect homeless people to health care before their illnesses become critical.
