Health care coverage is available on the first day of regular employment for employees
scheduled to work 30 or more hours per week. Employees may also cover their eligible
dependents. Included are their spouse or domestic partner, unmarried dependent children
under age 19, or unmarried dependent children under age 26 if they are full-time students.
Depending on where you live, you have up to three medical plan options from which to
choose. Your choices include:
If you are located in an area that is not served by any of the plans, you may select the
Out-of-Area Plan.
Enrollment
You can enroll in this benefit within the first 31 days of employment or at Open
Enrollment. You may add dependents within 31 days of a life event change, such as
marriage, birth or adoption. You can also waive health care coverage altogether, which
may be an option to consider if you have coverage under another health plan.
Plan Costs
You and Seagate share the cost of health care. When you purchase medical coverage, you
and your enrolled dependents also receive:
Dimensions Exclusive
The Dimensions Exclusive plan, administered by United HealthCare (UHC), offers you a
network of conveniently located health care providers, hospitals and clinics from which
to choose. You may receive care from anyone in the network at any time without referrals.
If you seek care from providers who are members of the UHC Options PPO network, most
services are covered at 100%. Certain services may require a small copayment at the time
of service. In-network providers file claims and all necessary paperwork is completed for
you.
You may see a provider who is not in the network, however, most eligible charges will
be covered at 50% (with no out-of-pocket maximum). The Dimensions Exclusive plan is
designed to encourage you to receive in-network services.
Prescription Drug, Employee Assistance Program, and Vision Coverage
When you enroll in medical coverage, you also receive prescription drug, employee
assistance, dental and vision coverage.
Dimensions Preferred
The Dimensions Preferred plan, administered by United HealthCare (UHC), offers you the
option to seek in-network or out-of-network care whenever you wish. The UHC Options PPO
network includes numerous health care providers, hospitals and clinics. If an in-network
provider does not fit your needs, you may go out-of-network at any time. You maintain
complete freedom of choice each time you or a covered dependent seeks medical care.
Benefits are higher when you go in-network. The Plan generally pays 90% of eligible
in-network expenses and 70% of eligible out-of-network expenses (after you pay a $200
deductible). There are many advantages to seeing an in-network provider. In-network care
is covered at 90%. (Certain services may require a small copayment at the time of service.)
And, your in-network provider will file your claims for you and complete any necessary
paperwork. You also reduce your out-of-pocket expenses.

assistance, dental and vision coverage.
Out-of-Area Plan
This medical plan is available to employees who live outside UHC's network. The
Out-of-Area Plan is a traditional plan that requires you to first pay a $200 deductible,
and then you and the Plan share costs. The Plan generally pays 80% of eligible expenses.

When you enroll in medical coverage, you also receive prescription drug, employee
assistance, dental and vision coverage.
The HMOs
HMOs are available to employees in California, Minnesota, Oklahoma and Colorado. HMOs are
managed care programs that offer comprehensive medical benefits to people who live in a
specific service area. You must use HMO providers in order to receive benefits. Typically,
you must choose a Primary Care Physician who is responsible for coordinating all your
medical care. Most services are covered at 100% after a copayment. Specific information on
the HMO offered in your area can be obtained from your local Benefits Representative. HMOs
available in each area are:
Northern California - HealthNet and Kaiser
Southern California - Kaiser
Colorado - Kaiser
Minnesota - HealthPartners
Oklahoma - Prudential
When you enroll in an HMO, you and your enrolled dependents automatically receive
prescription drug and vision coverage through your HMO plan. You also receive employee
assistance program coverage through United Behavioral Health and dental coverage through
MetLife.
Prescription Drug Program
The Prescription Drug Program is automatically provided to employees and dependents who
enroll in the Seagate Dimensions Exclusive or Preferred medical plans. If you enroll in
an HMO, you will receive your prescription drugs through your HMO's pharmacy.
Employee Assistance Program
The Employee Assistance Program, (EAP), administered by United Behavioral Health (UBH),
is available to you and your eligible dependents, as long as you're enrolled in a Seagate
medical plan. The EAP can help you with a variety of issues, such as chemical dependency,
marital and family issues, work stress and other life issues. The EAP provides five
confidential counseling sessions per family member per calendar year covered at 100%. If
you need additional sessions and are enrolled in the Dimensions Exclusive or Preferred
plans, you can continue to see the same UBH provider with coverage at the in-network
benefit level. If you are enrolled in an HMO, you must call your HMO to seek care within
the network.
Dental
The Seagate Dental Plan, administered by MetLife, covers all employees and dependents who
enroll in any of the Seagate medical plans (including HMOs). Dental coverage is effective
on the first day of enrollment in a medical plan. The Dental Plan lets you see any dental
provider you wish, however, you will receive a higher level of coverage when you see a
dentist who participates in the MetLife Preferred Dental Program (PDP). And all PDP
dentists will file your claims for you.
The Plan pays a higher percentage of the cost for all services, including 100% for
preventive care (with no deductible) and 90% for minor restorative care. You take
advantage of additional savings because participating dentists accept negotiated fees as
payment in full.
Vision
All employees and dependents who enroll in the Seagate Dimensions Exclusive and Preferred
medical plans are covered for routine vision care through the Vision Service Plan (VSP).
If you enroll in an HMO, vision coverage is provided through the HMO.
VSP provides coverage for vision exams, lenses, frames and contacts. You are free to see
any vision provider, however, you receive the highest benefit when you see a VSP provider.
When you see a VSP provider, VSP will handle all paperwork for you. If you see a non-VSP
provider, you'll need to submit your receipts for reimbursement along with a non-member
claim form. VSP's provider network is extensive, with at least two providers located
within 10 miles of most Seagate employees.
