Just in case this registration form looks really scary to you, here are some easy-to-follow guidelines (please note: the new deadline for submitting a fee waiver request is March 22th, the early registration deadline):

  • Everybody turns in Part A, which includes the Activity Release.
  • If you are under-18, please have your parent or guardian fill out Part B and also request a chaperone to fill out Part C. Chaperones must fill out Part A as well.

  • Chaperones are not required to be with their students at all times, but they are expected to be responsible for their whereabouts (and no, you can't just say "they're around").
    •
  • If you need a fee-waiver, please return Part D no later than March 22, 2001. If the registration fee is an issue, you may want to consider applying for a fee-waiver.

  • If you need information on hotels and other lodging, please answer "yes" to that question on the Registration Form and we'll send you more information.
    •
  • We will have peer facilitators leading small group discussions. If you want to facilitate, please answer "yes" to that question on the Registration Form and we will contact you with more information and future training dates. Answering "yes" does not commit you to be a facilitator.

  • Pre-registration deadline (received by March 22, 2001): $25-Student / $30-General
  • On-site/late registration deadline: $30-Student / $35-General
