Non-serving personnel can load and print the Medal Application Form

Next of kin of deceased ex-service personnel should also load, print and complete the Certificate of Kinship. Information and medals can only be released to the direct legal next of kin. This form could also be used for the legal next of kin to give their permission for information or medals to be released to a different family member or family friend. A photocopy of the relevant death certificate should also be submitted. The completed application form should be posted to:
