The annual Confidential Financial Statement (CFS) is a group of financial documents prepared at the end of the fiscal year by all parishes of the Archdiocese of Milwaukee. The data contained in these reports is used to calculate the archdiocesan assessment as provided for in Canon 1263 of the 1983 Code of Canon Law, for strategic planning purposes, for reporting to the NCCB, for proxy determinations, and for other analytical purposes. It is confidential in that it is not public record.

To facilitate the preparation of the report at the parish level and to expedite the processing of the report at the diocesan level, the CFS package has been prepared using a Microsoft Excel linked workbook again this year. Microsoft Office is the standard business software for the Archdiocese of Milwaukee.

Access a blank copy of the Confidential Financial Statement Excel workbook by clicking on the 'Download' icon below. You will be prompted to open or save the workbook to disk; select the Save to Disk option. Save the empty workbook to your computer hard drive or a network drive.

The Excel workbook is made up of several worksheets identified by tabs across the bottom of the screen. Instructions for completing the form are contained in the first worksheet. All information is entered on the Data Entry and Explanations worksheets. All other worksheets are populated from the information entered. All worksheets are password protected so that you can only enter information in fields that require information. Some fields are edited for valid data. You may view the formulas for any cell, but you will not be allowed to change protected cells or enter information directly to any of the report pages. You can print any of the worksheets.

After completing the workbook, save a copy for your records, then attach a copy to an e-mail addressed to parishfinance@archmil.org. Completed forms are due by Friday, September 15th. Information in this report is used to calculate the parish assessment. Parishes that do not submit Confidential Financial Statements by the deadline will be billed an estimated amount that includes a penalty.
