  • Continuity: When the Operative is reviewing Mal's file, Mal's first name is first shown spelled "Malcom", then "Malcolm".

  • Continuity: After Jayne passes to Simon the Blue Sun jug of booze, and Simon picks up to drink, the label has rotated away from the viewer.

  • Continuity: A security camera feed shows Mal and Jayne sitting down at a table with Fanty and Mingo before they enter the bar (before the dialog between Mal and Kaylee).

  • Continuity: The wide angle shot of the security camera's feed when Mal and Jayne first enter the bar shows a time code stamp which is repeated when the shot is zoomed in for a closer look.

  • Continuity: In the very last scene when River and the Captain are flying the ship, Mal presses a button causing it to light up. In the next shot the light is off, and in the next shot it is back on again.

  • Continuity: At the start of Dr. Caron's holographic recording, there is a noticeable delay before she says, "...and you can see, it isn't what we thought". When the recording is replayed and broadcast at the end, the delay is gone.

  • Continuity: When the wall is blasted after River kills all the Reavers, you can see a dead Reaver flinch from the explosion in the bottom left-hand of the screen

  • Factual errors: After Malcolm returns from Mr. Universe's generator shaft and the blast doors open, there is surprisingly little blood on River's clothes, despite the dozens of Reavers she butchered with her axe and machete.

  • Continuity: In the records room scene when the operative says, "Stop, Backtrack, Stop!" you notice that River's eyes are pointing straight. When the scene changes, and the Operative emerges from the record projector, River's eyes are pointed to her left.

  • Revealing mistakes: When Kaylee comes across the body of a 'presumably' killed boy, if you look near the gun shot to his tummy you'll see that he's breathing.

  • Miscellaneous: Most of the written "Chinese" on sets throughout the movie is in fact nonsense. For example, a pod marked "Search and Rescue" in English bears a string of characters that mean "hillock - thus - for - it - minor."

  • Revealing mistakes: In the scene directly after Simon being wounded, when River somersaults through the hatch, you can see a knee brace on her right knee. The brace disappears in the next shot.

  • Crew or equipment visible: In the vault scene, when Jayne comes running down the stairs to tell Mal that Reavers are in town, you can see the set on the left of the shot.

  • Continuity: In the vault scene, three people can be seen running past Mal to get out of the vault, whereas only Jayne and Zoe actually left.

  • Revealing mistakes: In the scene at the temple, when the operative and Mal are fighting, the operative kicks his gun from his hand. You can clearly see it's a stunt double he knocks the gun from, and not Fillion.

  • Continuity: When Simon rescues River at the start of the film, he smudges the spot of blood on her forehead. When they leave the room the spot is no longer smeared.

  • Continuity: On Miranda around the time of River's 'make me a stone' speech Jayne is turning around to the left, in the next shot he is turning around to the right. In the commentary for the special edition he admits this was on purpose.

Goofs below here contain information that may give away important plot points. You may not want to read any further if you've not already seen this title.

  • Continuity: SPOILER: When the dying Shepherd Book removes his blood-covered hand from Mal's face, there is no blood left on Mal's cheek, but there is smudged blood in the next shots.

  • Crew or equipment visible: SPOILER: In the closing scene where the Operative tells the Alliance soldiers to stand down, one of the soldiers (almost out of frame) lowers a set microphone.

  • Continuity: SPOILER: The weapon Inara uses in the final shoot-out changes in several shots - when she's in the background of a shot, she's using a bow-and-arrow, but when she's in the foreground, she's using a more high-tech-looking weapon referred to by Joss Whedon in his commentary as a "bolt gun". According to the commentary, Inara's bow-and-arrow simply did not have a very high visual impact, and did not seem to be the sort of weapon that Inara would use, so the weapon was changed in close-up shots using computer-generated images. For distant shots, they simply re-colored her bow so that it was harder to see, and removed the bowstring where it crossed her face, but it is still possible to see the bow in several shots. This change explains the strange hold that Inara uses on her "bolt gun", as well as explaining why she appears to fire by releasing the hand-grip.

  • Revealing mistakes: SPOILER: On Miranda, when River is doing her "Please God make me a stone..." speech and Simon goes to comfort her, his eyes follow the camera rotating around them. He looks directly at it for a split second before it pans up to Jayne.

  • Continuity: SPOILER: While being chased out of town by reavers, the mule wipes out a market stall and its yellow awning. As the reavers then pass the same spot, the awning and market stall are still intact and in place.

  • Factual errors: SPOILER: Inarra states that the planet Miranda is "the furthest planet out." If that is the case, then a view of the solar system's star from Miranda should be a very distant light. However (for thematic reasons), Miranda is awash with bright light, as though it were, instead, the closest planet to its star.
